    I have uploaded all the handouts for class 1 to the chat and message board.
    The exemption is the Patient Tracking form. I am uploading a Excel workbook for each student. The patient tracking form is a new assignment for this course and is going to be a trial run for other classes. This will provide me with information regarding the types of patients you are seeing in the clinical area. it should also be a tool for you to use when interviewing for positions to provide information regarding your clinical experiences.

  • Employment Contract Resources

    Included are examples of NP contracts to help you with formulating your contract. This should represent your ideal job. Some are very simple and some are very detailed. I have also included an "independent contractor" agreement should you need one in the future. Remember, the NYS Collaborative Practice Agreement is an entirely different document and the 2 should not be confused. Please review these documents before class and be ready to discuss as per the course schedule.
